Advanced Care Planning for Primary Care Providers

As the first point of contact for patients and their families, Primary Care providers play a vital role in guiding advance care planning and palliative care. In this presentation, Brett Snodgrass, FNP-C, CPE, ACHPN, FAANP, will discuss how to effectively communicate with seriously ill patients and how to utilize the 5 Wishes Form. This foundation-building presentation is for any primary care provider who wants to further develop their advance care planning and palliative care skills.
